Certification Exam Registration Form

IMPORTANT: If requesting exam accommodations, supporting documentation must be submitted to CertificationExams@gov.nl.ca a minimum of 3 weeks prior to the exam date chosen.

EXAM DATE INSTRUCTIONS: If requesting ACCOMMODATIONS other than "Extended Time", a different exam date and location may be sent to you than the dates listed below.

IMPORTANT:

• Registration forms will not be accepted after the deadline date in that location.
• A rewrite will be permitted a minimum of thirty (30) calendar days after the previous exam writing.
• If you are unable to attend an exam, you must email CertificationExams@gov.nl.ca at least three (3) business days prior to the exam date.
• For no-shows, you must send an email with supporting rationale before a new date will be scheduled.

FORM SUBMISSION INSTRUCTIONS:

• Upon submitting this form, you will receive a message that we have received your form. No need to email us for further confirmation at this point.
• The form will be processed to determine eligibility, and you will be notified if you are not eligible to write.
• If approved, you will receive an email approximately one week before the exam with a CONFIRMED EXAM DATE, location and time.
