Application for Special Permit

Section 1:
Provide training and certify that all drivers operating the vehicle have taken a mandatory driver training course and are qualified to operate the trailer. The training must include instruction on:
• When the lift axle should be deployed (down)
• When the lift axle may be in the raised position
• What to do if the lift axle fails to function
(b) Maintain training files on drivers operating the vehicle.

Section 2:
(a) Operate only those vehicles authorized under the Quad Axle Semitrailer Permit; and
(b) Retain special permit in vehicle and produce, at the request of a Peace Officer or Traffic Officer, the Quad Axle Semitrailer Permit.

DGSNL reserves the right to suspend or cancel, without notice, a permit, for non-compliance with the conditions placed upon the permit holder.

If there are any changes in limitations based on this application, the permit may have to be refunded and resubmitted as a new application.
