Complete the Driver/Photo ID Application form

If your driver’s licence or Photo ID is lost, stolen, or damaged, you can apply in person, by phone or in writing by mail or fax, to the nearest Motor Registration Division office in your area for a replacement. If you know your driver’s licence number, please provide it with your request. If you do not, the following information is required:

    2. date of birth.
