How to Apply

If you or someone you care for needs help with daily living, NL Health Services can work with you to find the right support. This may include help at home, in a personal care home, or long-term care. Here’s how the process works:

Clinical Assessment
A health care professional will talk with you and complete a clinical assessment. This helps figure out:

  • What care you need
  • What services will best support you

They will review:

  • How independently you can manage daily tasks like bathing, dressing, cooking and moving around
  • Your overall health
  • Any safety concerns
  • What supports you already have in place, such as help from family, friends, or other  services

Staff will work with you and your family to decide which services are the best fit for your needs.

Financial Assessment
You may request a financial assessment to determine:

  • If you qualify for a subsidy
  • How much you will be required to pay toward your care, if any

The assessment:

  • Is based only on your yearly income
  • Does not look at savings, property, or other assets
